Members pay a $200 deductible for outpatient medical services. Assurant Insurance will pay up to 80% of the covered charges up to $500 per calendar year, the member pays 20% of the covered charges. Non Surgical and non facility inpatient services are not considered under the inpatient per day maximum,requiring coinsurance. Pre-existing conditions are only covered when member has been continuously insured under this plan for 12 months.
